Why they were pulled: three kinds of failure

Failed dissolution leads, behind the Diovan, both metoprolol, both amlodipine-olmesartan and the chlorthalidone recalls. The test times how fast a tablet releases its drug in a lab bath; fail it and the dose on the bottle may not be the dose your body receives.

Nitrosamines are the second group. Bisoprolol-HCTZ, prazosin and 80 mg furosemide each exceeded the limit for a drug-specific N-nitroso compound, the impurity family behind the 2018 valsartan recalls. The FDA publishes acceptable intake limits for them.

The remaining four reasons, a foreign substance, an unapproved raw material, an out-of-range impurity test and a wrong imprint, each apply to a single product.

Lot numbers and NDC codes for each entry sit in the FDA’s weekly enforcement reports. The Teva metoprolol recall alone covers 112,327 bottles; the clonidine patches, 300,133 cartons.

What Class II means when it is your prescription

Under the FDA’s recall definitions, Class II means possible temporary or medically reversible harm, or a remote chance of serious harm. Class III means adverse effects are unlikely.

The stop-immediately tier is Class I, and no 2026 blood pressure entry we found sits there.
